New 2015 Tax Forms for Health Insurance

You will be receiving a new tax form: the 1095-B, from the Washington State Health Care Authority (HCA) or the 1095-A from the Health Benefit Exchange (HBE). The 1095-B form was mailed January 15, 2016 and will show that you met the minimum essential health care coverage in 2015. The 1095-A Health Insurance Marketplace Statement will arrive in early February and is required for those who enrolled in a health plan through the Washington Healthplanfinder in 2015.

Please keep these forms for your records, as you will need it to complete your 2015 tax return.

Some members might receive more than one tax form depending on the type of health coverage they had in 2015 and on their employer.  Examples are:

  • The 1095-A is given out by the Health Benefit Exchange.
  • The 1095-B can also be sent by other government-sponsored programs, employer-sponsored group health plans, and more.
  • State employees and retirees enrolled in the Uniform Medical Plan through the Public Employees Benefits Board (PEBB) Program, as well as employees of certain employers, will receive the 1095-C form from their employer or the Health Care Authority (HCA).
